Massachusetts Regulations for Immunizations
Compliance with the Massachusetts regulations for immunizations is required to attend our camp (provided below).
For children younger than age 5
- DTaP- 4 doses
- Polio/IPV- 3 doses
- Hepatitis B- 3 doses or evidence of immunity
- Hib- 1-4 doses
- MMR- 1 dose (given on or after 1st birthday) or evidence of immunity
- Varicella- 1 dose (given on or after 1st birthday) or evidence of immunity

For children older than age 5 and younger than age 12
- DTaP- 5 doses (4 are only acceptable if the 4th dose was given on or after 4th birthday)
- Polio/IPV- 4 doses (3 are acceptable if the 3rd dose was given on or after 4th birthday)
- Hepatitis B- 3 doses
- MMR- 2 doses or evidence of immunity
- Varicella- 2 doses or evidence of immunity
For children age 12 and older
- TDaP- booster
- Polio/IPV- 4 doses (3 are acceptable if the 3rd dose was given on or after 4th birthday)
- Hepatitis B- 3 doses
- MMR- 2 doses or evidence of immunity
- Varicella- 2 doses or evidence of immunity
- Meningococcal A, C, W, Y
* If your child is scheduled for a physical exam during the summer months, please let the camp nurse know so she can make a note on your child’s profile.
* If your child has a history of varicella/chickenpox disease, please have the licensed professional document this information for our records.
Medical Forms
Required at time of Registration: Physician Health Form must be uploaded to your camper’s online account/dashboard at time of registration in order to complete your registration. This Physician Health Form must contain details on immunizations, in English, and signed by a Doctor.
June 1st Deadline
Updated Physician Health Forms and/or additional forms can be uploaded to your Camper’s online account at any time up until June 1st. Please note, that submission of forms after June 1st may result in the inability of your camper to attend camp on his/her first day as the Camp Nurse must review AND approve your medical forms before any camper can attend camp.
Camp Nurse Approval
Campers without adequate Health forms provided will not be permitted to attend camp. Please understand that evaluations of Health Forms are completed in the weeks prior to camp Opening Day. The Camp Nurse must review AND approve your Health Forms before your camper will be permitted to attend camp which may not be possible the same day you drop off your forms.
Late Submissions
After June 1st submissions can not be guaranteed to be processed in time for a camper’s first day of camp regardless of the date your camper is scheduled to start camp. The Camp Nurse will complete Medical Form reviews BEFORE Camp Opening Day so that she can concentrate on the health and well-being of the campers on campus each day during camp. Providing forms once camp opens is considered a LATE submission and as such cannot be guaranteed to be processed in time for a camper’s first day of camp regardless of the date your camper is scheduled to start camp.
Physician Health Form
This is your child’s medical record form as obtained from his/her doctor that details his/her immunization record.
- Families residing in the US: Physicians prefer to use their own Health forms for their patients rather than use a school or camp specific form. As a result, we do not provide a form for you to download. Please simply ask your Physician for a copy of his/her form. The form is reviewed by our Camp Nurse and checked for compliance with the Massachusetts immunizations recommendations.

Important info regarding the Physician Health Forms
- BBN Student: If your camper is a BB&N Student, you must still provide the Summer Camp with a copy of your child’s Physician Health Form as we do not access school records for our summer camp.
- Physician Health Forms are valid for one year from the date of the previous exam. It is required that an updated Physician Health Form be submitted at the time of the annual exam in order for the student to continue participation in Summer @ BB&N.

Additional Medical Information
Inhaler
If your child will require the use of an Albuterol Inhaler at camp, a Medication Order Form must be completed. This form is available on this page under “Links to Important Forms”. This form must be completed before your child’s attendance at camp. Please note that the Medication Order Form is required to be signed by a doctor, nurse, or a licensed prescriber. Once signatures are obtained, you can upload this document to your online Camper Dashboard.
Epi Pen
If your child will require the use of an epi pen at camp, a Medication Order Form must be completed. This form is available on your online Camper Dashboard to download or available below via the links. This form must be completed before your child’s attendance at camp. Please note that the Medication Order Form is required to be signed by a doctor, nurse, or a licensed prescriber. Once signatures are obtained, you can upload this document to your online Camper Dashboard. Parents are required to bring 2 labeled epi pens for each camper in need directly to the Camp Nurse at drop off on the first day of camp.
In addition, the appropriate Allergy Action Plan (regular or junior) as provided below must also be completed. These forms are also available via download on your online Camper Dashboard. Please complete these forms in their entirety and upload them onto online Camper Dashboard account at least two weeks prior to the start of camp.
Medication: With or Without Prescription
Medication Requiring a Prescription?
If your child requires ANY* prescription medication during their stay at camp (this includes Epi Pens and Inhalers). This form is available on this page under “Important info regarding the Physician Health Forms”. This form must be completed before your child’s attendance at camp. Please note that the Medication Order Form is required to be signed by a doctor, nurse, or a licensed prescriber. Medication Not Requiring a Prescription?
If your child requires ANY* non-prescription medication during their stay at camp, please be sure to fill out the appropriate questions on the online registration form when registering for summer camp. * Please note: non-prescription medication refers to all over-the-counter medication, including all cough and cold remedies.
Seizures
If your camper is prone to seizures or has ANY history of seizures regardless of how far in his/her past, please download the following form and complete it and then upload it back onto your online Camper Dashboard.
Sunscreen and Bug Spray
All campers should come to camp with sunscreen and bug spray already applied at home. We will not provide a second application of sunscreen or bug spray unless you have supplied us with the lotion/sprays and appropriate directions. All lotions/sprays from home should be marked with the camper’s first and last name. Please note: bug repellent containing DEET in concentrations of 30% or less is recommended for protection from insects.
Dogs on Campus
Please note, that our school campus is dog-friendly. However, any sensitivities to dogs will be taken into account during the camp day. Families visiting Summer@BB&N must have dogs leashed and under their supervision and control at all times. Dogs are not permitted in campus buildings.
